Senior Social Services
For far too many seniors, their later years are not “golden”. Because of this fact, for nearly four decades, Family Matters of Greater Washington has helped to keep elderly residents of the District of Columbia safe and comfortable in their later years through a broad range of professional services. Our Department of Senior Social Services works to ensure the financial, social and safety needs of older adults. We work to protect and empower the frail elderly so that they are able to remain active, independent residents of their communities. We currently provide a host of services and programs that offer community-based support aimed at promoting active and independent lifestyles for seniors. Our specific aging related goal is to enhance and sustain the quality of life of seniors, focusing special attention to those who are in greatest need. Our service design is driven by advocating, empowering and strengthening the older person; and putting benefits and other services in place that will enable them to live more active, independent lives.
To ensure the health and independence of seniors in our community we are proud to offer a broad range of services to older adults including:
Long Term Care Comprehensive Geriatric Assessment & Case Management
Our community-based long term care comprehensive assessment and case management service is designed to keep seniors safe and living independently in the community for as long as possible. Our systems of care focus attention to residents ages 60 and older who are at-risk of premature nursing home placement due to multiple problems with health, income insecurity and ability to manage normal activities of daily living. Through our geriatric comprehensive assessment, case management, counseling, and heavy house cleaning services, we are able to reduce the effects of aging and maintain the older person in the community for longer periods. Additional services include assistance with arranging for medical care, escort services, emergency grocery shopping, financial counseling, life support transportation, and applying for public and private benefits.
Adult Abuse Prevention
Every man, woman and child deserves to be treated with respect and to be safe from harm by those who come in contact with them from day to day. Many older persons find themselves victims of physical, emotional or psychological abuse, financial exploitation and often neglect. Family Matters has taken action against adult abuse. Our Adult Abuse Prevention service is funded by the District of Columbia Office on Aging and we work in partnership with other local public and private agencies addressing adult abuse prevention through education, counseling and abuse, neglect and even criminal prosecution.
Home and Community-Based Waiver for the Elderly
Medicaid Waiver provides case management to individuals ages 65 and over who reside in the District of Columbia, are eligible for Medicaid, earn less than $1,500 per month, and are suitable for nursing home placement but would prefer to live at home. We enable individuals to choose home-based care over nursing home facilities.
Saturday Alzheimer’s Respite Program
Since 1986 our respite program has aided Alzheimer’s patients and caregivers. Four Saturdays a month, Alzheimer’s patients come to our program for a crucial social and therapeutic outlet, while their caregivers receive educational support, and a much needed break from their responsibilities.
Emergency Assistance for Seniors
Family Matters provides emergency assistance and heavy house cleaning services to low-income seniors living in the District of Columbia who are in danger of being evicted from their homes due to non-payment of rent or failing to maintain the premises in a clean and sanitary condition. Staff also assists them in finding long-term solutions that enable them to remain independent.
For more than fifty years, we have been a proud leader in the field of geriatric social services. In 2007, our expertise as a senior service provider was recognized when we were chosen by the Alliance on Children and Families to serve as a mentor organization in the field of Geriatrics. All but the most recent additions to our Senior Social Services staff hold certifications in Geriatrics.
